Polymyxin B sulfate is a cyclic lipopeptide antibiotic produced by the bacterium *Paenibacillus polymyxa*. It is a small molecule drug primarily indicated for the treatment of serious infections caused by susceptible Gram-negative bacteria, particularly multidrug-resistant strains such as *Pseudomonas aeruginosa*. Developed and manufactured by ABL Antibióticos do Brasil for the hospital market, it acts as a detergent-like agent by binding to lipopolysaccharides (LPS) and phospholipids in the outer membrane of Gram-negative bacteria. This binding disrupts the integrity of the bacterial cell membrane, leading to increased permeability and subsequent cell death. Due to its potential for nephrotoxicity and neurotoxicity, its use is typically reserved for severe infections where other therapeutic options are limited or ineffective.
