This is a fixed-dose combination of three drugs: - **Sulfadoxine-pyrimethamine** (SP): an antimalarial combining a sulfonamide (sulfadoxine) and an antiprotozoal (pyrimethamine), which act synergistically by inhibiting two sequential steps in the folate synthesis pathway of Plasmodium parasites, thereby blocking DNA synthesis and parasite replication[7][9]. - **Amodiaquine** (AQ): a 4-aminoquinoline antimalarial that interferes with the detoxification of heme in Plasmodium species, leading to toxic buildup within the parasite[3]. - **Azithromycin**: a macrolide antibiotic with additional antimalarial activity; it inhibits protein synthesis by binding to the 50S ribosomal subunit. Azithromycin has been studied for its potential to reduce childhood mortality when co-administered with SMC regimens but did not show significant reduction in mortality or hospital admissions when added to SP+AQ for seasonal malaria chemoprevention[1][8]. The primary indication for this triple combination is as an experimental regimen for seasonal malaria chemoprevention (SMC) in children living in areas of high malaria transmission. The addition of azithromycin has also been investigated for its effects on nutritional status and all-cause child mortality but without clear benefit over standard SMC regimens[1][8]. The combination is not currently approved as a standard therapy but has been evaluated in large-scale clinical trials.
