SUN-627 is a brain-penetrant, small molecule antagonist of the P2X7 receptor currently under development by Sundance Biosciences for the treatment of progressive forms of multiple sclerosis (MS). The P2X7 receptor is an ATP-gated ion channel predominantly expressed on microglia in the central nervous system; its activation by extracellular ATP, which is released during tissue damage, triggers the assembly of the NLRP3 inflammasome and the subsequent release of pro-inflammatory cytokines such as IL-1β and IL-18. By blocking this pathway, SUN-627 is designed to attenuate the chronic neuroinflammation that drives neurodegeneration and disability progression in patients with non-active primary progressive multiple sclerosis (PPMS) and non-active secondary progressive multiple sclerosis (SPMS). The compound is currently being evaluated in a Phase 2a clinical trial to assess its safety, pharmacokinetics, and impact on CNS inflammatory biomarkers.
