Overview

**Supervised Decolonization** was a University of Pennsylvania household decolonization regimen evaluated in the randomized community MRSA-transmission study NCT00966446. It combined intranasal **mupirocin** ointment, administered twice daily for 7 days, with topical **chlorhexidine gluconate** body wash used on days 1 and 7; adherence was supported by daily telephone-call or text-message reminders. Mupirocin inhibits bacterial isoleucyl-tRNA synthetase and suppresses protein synthesis, while chlorhexidine is a membrane-active antiseptic that reduces bacterial burden on the skin. The regimen was studied in households of patients with community-onset methicillin-resistant *Staphylococcus aureus* infection to promote MRSA colonization clearance and reduce household transmission and recurrent skin and soft-tissue infection.
