Overview

SYCUME is a recombinant anti-insulin-like growth factor 1 receptor (IGF-1R) monoclonal antibody developed by Innovent Biologics for the treatment of thyroid eye disease (TED). It is the first IGF-1R antibody drug approved in China and only the second globally for this indication. IGF-1R is a transmembrane tyrosine kinase receptor involved in development, metabolism, and immune regulation. In TED, IGF-1R is overexpressed on orbital fibroblasts (OFs), B cells, and T cells. SYCUME works by blocking activation of the IGF-1R signaling pathway mediated by its ligands or agonistic antibodies, thereby reducing downstream inflammatory factors. This leads to decreased synthesis of hyaluronic acid and other glycosaminoglycans from OFs activation, alleviating inflammation and preventing OF differentiation into adipocytes or myofibroblasts—ultimately reducing disease activity and improving clinical symptoms such as proptosis (eye bulging), diplopia (double vision), orbital congestion, and edema. The approval was based on robust Phase 3 data showing significant efficacy in proptosis reduction with a favorable safety profile[2][3][5][9].
