SYM004 + nivolumab is an investigational combination therapy of two drugs, each with a distinct mechanism targeting cancer cells. **SYM004** is a 1:1 mixture of two recombinant chimeric IgG1 monoclonal antibodies (mAb992 and mAb1024) that target non-overlapping epitopes on the extracellular domain of the epidermal growth factor receptor (EGFR). SYM004 blocks ligand binding, promotes rapid internalization and degradation of EGFR, inhibits both ligand-dependent and ligand-independent EGFR activity, and induces antibody-dependent cellular cytotoxicity (ADCC) and complement-mediated cytotoxicity. It is designed to overcome resistance to other anti-EGFR monoclonal antibodies and has demonstrated activity in solid tumors, including those resistant to cetuximab. **Nivolumab** is a monoclonal antibody that targets the programmed death-1 (PD-1) receptor on T cells, blocking an immune checkpoint and enhancing T-cell mediated antitumor responses. The combination aims to leverage targeted EGFR inhibition with SYM004 and checkpoint blockade with nivolumab to achieve synergistic anticancer activity.
