Synthetic long peptide (SLP) neoantigen vaccines are a class of personalized cancer immunotherapies designed to stimulate a patient's immune system against tumor-specific mutations. These vaccines consist of synthetic peptides, typically 25–30 amino acids in length, that encompass neoantigens identified through genomic sequencing of a patient's tumor. Unlike short peptides, SLPs require processing by professional antigen-presenting cells (APCs), which allows for the presentation of both MHC class I and class II epitopes, thereby activating both CD8+ cytotoxic T cells and CD4+ helper T cells. This dual activation is critical for generating robust and sustained anti-tumor immunity. These vaccines are often administered with adjuvants like poly-ICLC to enhance the innate immune response and are being investigated across various solid tumors, including melanoma, glioblastoma, and pancreatic cancer.
