SYS6090 + bevacizumab is a combination therapy under clinical investigation for the treatment of advanced hepatocellular carcinoma (HCC). SYS6090, developed by CSPC Megalith Biopharmaceutical, is an antibody-drug conjugate (ADC) composed of a humanized monoclonal antibody targeting Glypican-3 (GPC3) linked to the cytotoxic payload monomethyl auristatin E (MMAE). GPC3 is a heparin sulfate proteoglycan that is significantly overexpressed in HCC cells, providing a selective target for the delivery of MMAE, which inhibits tubulin polymerization and induces cell cycle arrest and apoptosis. Bevacizumab is a recombinant humanized monoclonal antibody that binds to vascular endothelial growth factor (VEGF), preventing its interaction with receptors on endothelial cells and thereby inhibiting tumor vascularization. This combination strategy is designed to provide synergistic anti-tumor activity by combining GPC3-targeted cytotoxicity with VEGF-mediated microenvironment modulation. It is currently being evaluated in Phase Ib/II trials, often in conjunction with the PD-1 inhibitor toripalimab.
