T-cell-depleted donor lymphocyte infusion (TCD-DLI) is a specialized cellular therapy utilized as the first step in a 'Two-Step' reduced-intensity conditioning (RIC) allogeneic hematopoietic stem cell transplantation (HSCT) protocol. Developed by the Sidney Kimmel Cancer Center at Thomas Jefferson University, this approach is designed to treat vulnerable patients with various hematologic malignancies. In this regimen, donor lymphocytes are collected and depleted of T-cells (specifically CD3+ cells) to mitigate the risk of graft-versus-host disease (GvHD) while preserving the beneficial graft-versus-tumor (GVT) effect. The protocol involves administering the TCD-DLI followed by a targeted dose of cyclophosphamide to eliminate alloreactive cells, and subsequently infusing the hematopoietic stem cell graft. This strategy aims to provide a safer transplant option for older or infirm patients who may not tolerate standard myeloablative conditioning.
