T-cell receptor alpha-beta+ CD19+ depleted allogeneic peripheral blood-derived hematopoietic stem cells is an advanced cellular therapy product developed by the IRCCS Istituto Giannina Gaslini for the treatment of high-risk neuroblastoma and other pediatric malignancies. The therapy utilizes a sophisticated graft engineering process to selectively remove αβ+ T cells, which are the primary mediators of Graft-versus-Host Disease (GvHD), and CD19+ B cells, which are associated with post-transplant Epstein-Barr virus (EBV) complications. By retaining γδ+ T cells and Natural Killer (NK) cells, the graft maintains a strong graft-versus-tumor effect and provides early protection against infections. This approach allows for successful HLA-haploidentical hematopoietic stem cell transplantation (HSCT) in children who lack a matched donor, offering a curative option with significantly reduced transplant-related morbidity.
