T10B9 (specifically T10B9.1A-31 or MEDI-500) is a murine monoclonal antibody of the IgM kappa isotype directed against the alpha-beta (αβ) heterodimer of the T-lymphocyte receptor (TCR) complex. Unlike the mitogenic anti-CD3 antibody muromonab-CD3 (OKT3), T10B9 is nonmitogenic and induces minimal cytokine release, significantly reducing the severity of cytokine release syndrome. A distinguishing feature of T10B9 is its ability to deplete αβ T-cells while sparing gamma-delta (γδ) T-cells, which are thought to play a role in maintaining immunologic tolerance and contributing to graft-versus-leukemia effects. It has been investigated for the treatment of acute cellular rejection in renal transplantation, induction therapy in heart and kidney-pancreas transplantation, and as an ex vivo or in vivo T-cell depletion agent in bone marrow transplantation to prevent graft-versus-host disease (GVHD).
