T1144 is a 38–amino acid, third‑generation HIV‑1 fusion‑inhibitor peptide derived from the C‑terminal heptad repeat (CHR) region of gp41 and engineered from the T651 sequence (aa 626–673) to include a defined pocket‑binding domain that engages the deep hydrophobic pocket of the gp41 N‑terminal heptad repeat (NHR), thereby forming a highly stable six‑helix bundle and preventing fusion of viral and host cell membranes.[2][3][1] Compared with the first‑generation fusion inhibitor enfuvirtide (T20), T1144 displays substantially greater antiviral potency, a higher genetic barrier to resistance, and retains activity against many strains resistant to T20 and T1249.[2][3][1] Pharmacokinetic studies have shown a markedly prolonged plasma half‑life in animals and humans relative to T20, and sustained‑release/long‑acting formulations of T1144 are being developed for chronic HIV treatment by Bioventures Holdings and partners such as Innovare and DelSiTech.[2][4][7][8]
