This quadruple therapy regimen combines Transarterial Chemoembolization (TACE) with the PD-1 inhibitor tislelizumab, the multi-kinase inhibitor lenvatinib, and the non-selective beta-blocker carvedilol. It is specifically designed for patients with unresectable hepatocellular carcinoma (HCC) and concurrent cirrhotic portal hypertension. Tislelizumab enhances the immune response against tumor cells by blocking the PD-1/PD-L1 pathway. Lenvatinib inhibits multiple receptor tyrosine kinases involved in angiogenesis and tumor growth, including VEGFR and FGFR. Carvedilol is used to manage portal hypertension and may potentially modulate the tumor microenvironment to enhance immunotherapy efficacy. TACE provides localized tumor control through embolization and chemotherapy.
