Overview

Tacedinaline + gemcitabine is a combination regimen of two anticancer agents: tacedinaline (also known as CI-994), an oral **class I histone deacetylase (HDAC) inhibitor**, and **gemcitabine**, a nucleoside analog that inhibits DNA synthesis. Tacedinaline inhibits HDAC1 selectively, promoting acetylation of histones and altering gene expression, leading to cell cycle arrest, apoptosis, and enhanced anti-tumor immunity. Gemcitabine is phosphorylated intracellularly and incorporates into DNA, causing chain termination and inhibition of DNA synthesis. This combination has been investigated primarily in advanced solid tumors, especially **pancreatic cancer**, where preclinical models suggested potential synergistic effects. However, in clinical trials the combination did not show improvement in overall survival or response rate compared to gemcitabine alone and was associated with increased hematologic toxicity such as neutropenia and thrombocytopenia[1][3][7].
