Tadalafil is a selective phosphodiesterase 5 (PDE5) inhibitor used to treat erectile dysfunction (ED), benign prostatic hyperplasia (BPH), and pulmonary arterial hypertension (PAH). It works by blocking the enzyme PDE5, leading to increased levels of cyclic guanosine monophosphate (cGMP), which causes smooth muscle relaxation and increased blood flow in specific tissues. For ED, this results in improved erectile function; for BPH, it relaxes muscles in the prostate and bladder; for PAH, it relaxes blood vessels in the lungs to lower blood pressure. Tadalafil was originally developed by ICOS Corporation and later co-developed with Eli Lilly. It is marketed under several brand names including Cialis for ED/BPH and Adcirca for PAH[1][3][4][5][8].
