This combination therapy consists of the phosphodiesterase-5 (PDE5) inhibitor tadalafil and a standard influenza vaccine, being investigated by the Ottawa Hospital Research Institute for use in the perioperative period for patients undergoing surgery for abdominal cancers. The regimen is designed to combat the profound immune suppression that typically follows major surgery, which can facilitate the spread of micrometastases. Tadalafil works by inhibiting myeloid-derived suppressor cells (MDSCs), which otherwise suppress natural killer (NK) cell activity. Simultaneously, the influenza vaccine acts as an immune stimulant to directly activate NK cells. By maintaining and enhancing NK cell function during the critical surgical window, the therapy aims to reduce the risk of cancer recurrence and metastasis.
