TAG fish oil refers to "triacylglycerol" (triglyceride) form of fish oil, which is the natural molecular structure of omega-3 fatty acids as found in whole fish. In this form, the omega-3 fatty acids eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) are esterified to a glycerol backbone. This distinguishes it from ethyl ester or re-esterified forms used in some supplements and pharmaceuticals. TAG fish oils are commonly marketed as dietary supplements for their content of EPA and DHA, which have been shown to reduce blood triglycerides, exert anti-inflammatory effects, and support cardiovascular health[1][4][10]. The mechanism of action involves reduction of hepatic production of very low-density lipoproteins (VLDL), decreased synthesis/secretion of triglycerides by the liver, modulation of inflammatory pathways via free fatty acid receptor 4 agonism and nuclear factor NF-kappa-B regulation[4], as well as direct effects on cardiac ion channels that may contribute to antiarrhythmic properties[4]. TAG fish oils are not prescription drugs but rather dietary supplements; prescription products use concentrated or modified forms.
