TAK-573 + cyclophosphamide + dexamethasone is an experimental combination regimen under investigation for the treatment of relapsed/refractory multiple myeloma (RRMM). TAK-573 (modakafusp alfa) is a first-in-class immunocytokine designed to deliver attenuated interferon alpha-2b to CD38-positive cells, resulting in both direct cytotoxic activity and immune system modulation. Cyclophosphamide is an alkylating agent that crosslinks DNA, leading to cell death, and dexamethasone is a synthetic glucocorticoid that exerts anti-inflammatory and immunosuppressive effects and is commonly used in myeloma regimens. The combination is in clinical evaluation for multiple myeloma, though most available studies focus on TAK-573 alone or in combination with dexamethasone and not routinely with cyclophosphamide. The mechanism of action for the combination is presumed to include targeted immunotherapy, cytotoxic chemotherapy, and immunosuppression.
