Talabostat + cisplatin is a **combination therapy** consisting of talabostat, an oral small-molecule inhibitor of dipeptidyl peptidases (notably fibroblast activation protein [FAP] and CD26), and cisplatin, a platinum-based chemotherapy. Talabostat’s mechanism includes immunostimulatory effects: it upregulates cytokines and chemokines, thereby enhancing both innate and acquired T-cell immunity. In preclinical models, talabostat enhances the activity of cisplatin, and clinical trials have shown synergistic antitumor effects in some melanoma models. However, phase II trials in metastatic melanoma reported that the combination did not significantly improve outcomes over cisplatin alone, though some durable partial responses and stable disease were observed.
