Tamalakyadi decoction

Tamalakyadi decoction (TD), also known as TMD12, is a classical polyherbal Ayurvedic formulation originating from the traditional Sri Lankan medical text *Sarasankshepaya*. It is composed of 12 plant ingredients: *Phyllanthus niruri*, *Terminalia chebula*, *Premna herbacea*, *Piper retrofractum*, *Piper longum*, *Solanum trilobatum*, *Tinospora cordifolia*, *Zingiber officinale*, *Piper nigrum*, *Solanum indicum*, *Solanum xanthocarpum*, and *Adhatoda vasica*. The formulation is traditionally used to treat respiratory and nasal conditions, including allergic rhinitis, asthma, and cough. Rather than targeting a single molecular pathway, Tamalakyadi decoction acts broadly on the immune and inflammatory responses of the nasal mucosa to airborne allergens, leveraging the synergistic phytochemical and antioxidant properties of its constituents. Researchers at the Institute of Indigenous Medicine and the Department of Pharmacology, Faculty of Medicine, University of Colombo, Sri Lanka, have developed and evaluated both the traditional liquid decoction and a ready-to-use freeze-dried powder formulation (FDF-TD) in a Phase II randomized, open-label, non-inferiority clinical trial comparing them to the antihistamine loratadine in adults with allergic rhinitis.
