Tamoxifen + tocotrienol-rich fraction is a combination therapy consisting of tamoxifen, a selective estrogen receptor modulator (SERM), and a tocotrienol-rich fraction (TRF) derived from vitamin E. Tamoxifen acts primarily as an estrogen receptor antagonist in breast tissue, inhibiting the growth of estrogen receptor-positive breast cancer cells. Tocotrienols are members of the vitamin E family with demonstrated antiproliferative and proapoptotic effects in preclinical studies. The combination has been investigated for synergistic anti-cancer activity, particularly in early-stage, estrogen receptor-positive breast cancer. Clinical trials have evaluated its efficacy as adjuvant therapy over five years; while some data suggest possible reductions in mortality and recurrence risk compared to tamoxifen alone, these findings were not statistically significant[1][3][4]. The mechanism involves both hormonal modulation by tamoxifen and potential immune enhancement or direct tumor inhibition by TRF.
