Tamoxifen + venetoclax is an investigational combination therapy consisting of two small molecule drugs with distinct mechanisms of action. Tamoxifen is a selective estrogen receptor modulator (SERM) that competitively inhibits estrogen binding to the estrogen receptor, thereby blocking the growth-promoting effects of estrogen in hormone receptor-positive breast cancer cells. Venetoclax is a potent and selective inhibitor of BCL2, an anti-apoptotic protein that helps cancer cells evade programmed cell death. The combination has been studied in patients with estrogen receptor (ER) and BCL2-positive metastatic breast cancer, where it demonstrated a tolerable safety profile and notable clinical activity, including radiologic responses and clinical benefit rates. This regimen targets both hormonal signaling pathways (via tamoxifen) and apoptotic resistance mechanisms (via venetoclax), aiming to enhance antitumor efficacy in ER+/BCL2+ tumors[1][2].
