Overview

Tandospirone is an anxiolytic and antidepressant medication primarily used in Japan and China, marketed under the brand name Sediel. It is a small molecule drug classified as a third-generation antianxiety agent with fewer side effects compared to earlier agents. Tandospirone acts as a potent and selective partial agonist at the serotonin 5-hydroxytryptamine 1A receptor (5‑HT1A receptor), with high affinity for this target and much lower affinity for other serotonergic, adrenergic, dopaminergic, or muscarinic receptors. Its mechanism involves activation of presynaptic 5‑HT1A autoreceptors in the raphe nuclei (full agonist) and postsynaptic 5‑HT1A receptors in forebrain regions (partial agonist), leading to inhibition of neuronal activity via G-protein-coupled inwardly rectifying potassium channels. Clinically, it is approved for generalized anxiety disorder and has been investigated for depression, schizophrenia adjunctive therapy (for cognitive symptoms), stress-induced anxiety-like behavior, visceral hypersensitivity associated with irritable bowel syndrome, and potentially other neuropsychiatric conditions[2][4][5][6][7].
