Overview

**Tapencarium (RZL-012)** is a novel injectable synthetic small molecule (5-(3,6-dibromo-9H-carbazol-9-yl)-N,N,N-trimethylpentan-1-aminium chloride) with cytolytic properties that disrupts adipocyte cell membrane integrity, leading to fat cell death and reduction in subcutaneous fat volume via the normal inflammatory and wound-healing process. Developed for aesthetic and fat-related disorders, it is administered in a single injection session into targeted fat deposits, such as submental fat (SMF), flanks, or areas affected by conditions like Dercum's disease and lipedema, with clearance of dead fat tissue observed by 84 days in studies. Phase 2b trials demonstrated significant dose-dependent improvements in SMF appearance (86% of high-dose patients achieved ≥1-grade improvement vs. 56% placebo) and MRI-measured fat volume reduction (68% responders with ≥10% reduction in high-dose group), with common but transient adverse events including injection site reactions, swelling, dysphagia, and mild nerve issues that resolved without sequelae.[1][2][3]
