Overview

Tarlatamab + carboplatin + etoposide is an investigational combination regimen being evaluated for the treatment of relapsed extensive-stage small cell lung cancer (ES-SCLC). The combination consists of tarlatamab, a half-life extended bispecific T-cell engager (BiTE) that targets delta-like ligand 3 (DLL3) on tumor cells and CD3 on T cells, alongside standard platinum-based chemotherapy (carboplatin and etoposide). Tarlatamab redirects cytotoxic T cells to DLL3-expressing cancer cells, which are prevalent in SCLC. Carboplatin and etoposide are cytotoxic agents that induce DNA damage and inhibit topoisomerase II, respectively. This specific triplet is being investigated in the REPLAT trial as a rechallenge strategy for patients who have previously progressed on tarlatamab monotherapy, based on the hypothesis that chemotherapy may synergistically enhance T-cell infiltration and modify the tumor microenvironment to overcome resistance.
