tarlatamab + etakafusp alfa is an investigational combination therapy being developed by Amgen in collaboration with Asher Biotherapeutics for the treatment of extensive-stage small cell lung cancer (ES-SCLC). The regimen combines tarlatamab (IMDELLTRA), a bispecific T-cell engager (BiTE) targeting delta-like ligand 3 (DLL3), with etakafusp alfa (AB248), a CD8+ T cell-selective interleukin-2 (IL-2) immunotherapy. Tarlatamab works by redirecting T cells to engage and lyse DLL3-expressing tumor cells, a common marker in SCLC. Etakafusp alfa is designed to selectively activate CD8+ effector T cells without stimulating regulatory T cells or other cells associated with IL-2 toxicity, thereby potentially enhancing the durability and magnitude of the T-cell response initiated by tarlatamab. The combination is currently being evaluated in the Phase 1b DeLLphi-311 clinical trial.
