**Tarlatamab + pembrolizumab** is an investigational combination therapy comprising tarlatamab, a bispecific T-cell engager (BiTE®) antibody, and pembrolizumab, a humanized monoclonal antibody targeting PD-1. Tarlatamab binds to delta-like ligand 3 (DLL3) on tumor cells and CD3 on T cells, redirecting T lymphocytes to induce cell-mediated cytotoxicity against DLL3-expressing tumor cells, primarily in small cell lung cancer (SCLC)[4][6]. Pembrolizumab, an immune checkpoint inhibitor, binds PD-1 on T cells and blocks its interaction with PD-L1 and PD-L2, reactivating exhausted T-cell responses and promoting antitumor immunity[2]. The combination strategy is currently studied in patients with relapsed/refractory small cell lung cancer (RR SCLC) and extensive-stage SCLC (ES-SCLC) who have progressed after platinum-based chemotherapy, with early-phase trials demonstrating a manageable safety profile and durable efficacy as first-line maintenance in ES-SCLC[1][3][7].
