Tarperprumig (ALXN1820) is an investigational bispecific antibody developed by Alexion Pharmaceuticals (AstraZeneca) for the treatment of diseases involving dysregulated complement alternative pathway activity. It is designed to bind both properdin and albumin, thereby inhibiting the function of properdin—a positive regulator of the complement alternative pathway—and extending its half-life via albumin binding. The primary indication under investigation is sickle cell disease (SCD), where excessive complement activation contributes to hemolysis and vaso-occlusive crises. Tarperprumig has shown efficacy in reducing complement hemolytic activity in healthy adults and is currently being evaluated in phase 2 clinical trials for SCD[1][3][4][8].
