Overview

Tauroselcholic acid (75Se), commonly known by the brand name SeHCAT, is a radiolabeled synthetic bile acid analog used as a diagnostic radiopharmaceutical. It is orally administered to investigate enterohepatic circulation, measure bile acid pool loss, and diagnose bile acid malabsorption (BAM) in patients presenting with chronic diarrhea, irritable bowel syndrome with diarrhea (IBS-D), or Crohn's disease. Chemically, it is a taurine-conjugated bile acid analog incorporating the gamma-emitting isotope selenium-75. Following oral administration, it behaves physiologically like natural conjugated bile acids, being absorbed in the terminal ileum via the apical sodium-dependent bile acid transporter (ASBT/SLC10A2) and excreted into the bile. Its retention in the body is measured over a seven-day period using a gamma camera, where a retention value of less than 15% indicates malabsorption.
