A neoadjuvant chemotherapy and targeted therapy regimen consisting of a taxane (docetaxel, paclitaxel, or nab-paclitaxel), carboplatin, trastuzumab, and pertuzumab. This combination, commonly referred to as TCHP or TCbHP, is being evaluated in the Phase III neoCARHP trial (NCT04858529) led by Guangdong Provincial People's Hospital for the treatment of HER2-positive early breast cancer. The regimen utilizes dual HER2 blockade via the monoclonal antibodies trastuzumab and pertuzumab, which bind to different domains of the HER2 receptor to inhibit downstream signaling and dimerization. This is combined with the cytotoxic effects of carboplatin, a platinum-based DNA alkylator, and a taxane, which stabilizes microtubules to prevent cell division. The trial compares this four-drug regimen against a de-escalated three-drug regimen (THP) to assess efficacy and toxicity.
