TB511 + pembrolizumab is an investigational combination therapy being developed for advanced solid tumors that are refractory or intolerant to standard treatments, including prior immune checkpoint inhibitor therapies. TB511 is a peptide drug conjugate (PDC) consisting of a tumor-associated macrophage (TAM)-binding peptide linked to an apoptosis-inducing peptide (dKLA); this molecule specifically targets M2-polarized TAMs in the tumor microenvironment, binds these cells, and induces mitochondrial apoptosis, leading to their depletion. Pembrolizumab is a monoclonal antibody that binds PD-1 on T cells and blocks interactions with PD-L1 and PD-L2, thereby promoting antitumor immune responses. The combination is designed to deplete immunosuppressive M2 macrophages and restore anti-tumor T-cell responses in patients with advanced solid tumors who are refractory or have relapsed after checkpoint inhibitor therapies. The main indications under clinical investigation are non-small cell lung cancer, prostate cancer, colorectal cancer, and hepatocellular carcinoma[1][2].
