Overview

TBA-354 is a synthetic small molecule belonging to the nitroimidazole class, developed as a next-generation antitubercular agent. It is structurally related to pretomanid (PA-824) and was designed to have improved potency, metabolic stability, and oral bioavailability compared to earlier nitroimidazoles such as delamanid and PA-824[1][4][7]. TBA-354 exhibits narrow-spectrum bactericidal activity against both replicating and nonreplicating Mycobacterium tuberculosis, with in vitro potency superior to PA-824 and comparable or better than delamanid[1][7]. The drug demonstrated exceptional efficacy in murine models of acute and chronic tuberculosis. However, clinical development was discontinued during Phase 1 trials due to mild but reversible neurotoxicity observed in human subjects[6][9]. The mechanism of action involves inhibition of mycolic acid biosynthesis through bioreductive activation within M. tuberculosis cells—a hallmark of the nitroimidazole class—leading to bacterial cell death[4].
