Overview

TBO-309 is a small molecule, potent, and selective ATP-competitive inhibitor of phosphatidylinositol 3-kinase beta (PI3Kβ) being developed by ThromBio Pty. Ltd. for the treatment of acute ischemic stroke. By targeting PI3Kβ, the drug specifically inhibits platelet activation, adhesion, and aggregation, particularly under conditions of disturbed blood flow, while also promoting platelet disaggregation. This mechanism allows for the inhibition of pathological thrombosis without significantly impairing normal hemostasis, potentially reducing the risk of symptomatic intracranial hemorrhage compared to traditional antiplatelet agents. It is currently being evaluated in the Phase II Co-STARS trial as an adjunctive therapy for patients with tandem occlusion undergoing endovascular thrombectomy and extracranial carotid artery stenting.
