TCF-1-overexpressing CD19.CAR-T is an experimental, autologous, CD19-directed CAR-T cell therapy in which human T cells are engineered to express a CD19-specific chimeric antigen receptor together with enforced overexpression of the T cell transcription factor TCF-1 (TCF7) to enhance a naïve/stem‑like phenotype, persistence, and antitumor potency against B‑cell malignancies.[2][7] By restoring or augmenting TCF-1 levels, these CAR-T cells exhibit increased proportions of less differentiated, stem cell–like subsets, reduced apoptosis, altered cytokine production consistent with lower cytokine release risk, and improved long-term cytotoxic functionality in preclinical models of relapsed or refractory hematologic cancers.[2][7] This construct is being explored in the research/preclinical setting as a next-generation CD19 CAR-T design aimed at overcoming limited persistence and exhaustion observed with conventional CD19 CAR-T therapies in leukemia and lymphoma patients.[2][6][7]
