This specialized cell therapy protocol, developed by St. Jude Children's Research Hospital, is an advanced hematopoietic cell transplantation (HCT) strategy for pediatric and young adult patients with high-risk hematologic malignancies. The procedure involves the use of a haploidentical donor graft that has been selectively depleted of TCRαβ+ T cells and CD19+ B cells using the CliniMACS device. This selective depletion is intended to prevent graft-versus-host disease (GVHD) while retaining TCRγδ+ T cells and natural killer (NK) cells, which provide essential anti-leukemic and anti-viral activity. Following the initial transplant, patients receive an infusion of CD45RA-depleted (memory) T cells to further enhance immune reconstitution without increasing the risk of GVHD. For patients with CD19+ malignancies, the protocol also incorporates the prophylactic use of blinatumomab, a bispecific T-cell engager, to target and eliminate residual disease and reduce the risk of relapse.
