TCR alpha beta T cell depletion is a cellular manipulation technique used in allogeneic hematopoietic stem cell transplantation (HSCT) to selectively remove donor-derived immune cells expressing the alpha/beta (αβ) subunits of the T-cell receptor (TCR) from grafts prior to infusion. The primary goal is to reduce the risk of graft-versus-host disease (GVHD), as αβ+ T cells are considered major mediators of this complication. This process preserves other beneficial immune cells such as gamma delta (γδ) T cells and natural killer (NK) cells, which contribute to anti-infective activity and graft-versus-tumor effects while minimizing post-transplant immunosuppression needs. The technique is especially valuable for patients lacking fully matched donors and has been applied in both haploidentical and HLA-matched transplants for malignant and non-malignant hematological disorders[2][3][6].
