Tebapivat is an investigational, oral small molecule that acts as a potent activator of pyruvate kinase (PK), including the red blood cell-specific isoform (PKR) and M2 isoform (PKM2). By activating PK, tebapivat enhances glycolytic pathway activity in erythrocytes, leading to increased adenosine triphosphate (ATP) production and improved red blood cell energy metabolism. This mechanism is intended to address anemia by improving red blood cell survival and function. Tebapivat is being developed primarily for the treatment of anemia associated with lower-risk myelodysplastic syndromes (LR-MDS) and sickle cell disease. It is currently under investigation in phase 2 clinical trials for these indications[6][7][9][10].
