Tebentafusp + durvalumab is a combination therapy under investigation for the treatment of metastatic cutaneous melanoma, particularly in patients who have progressed on prior checkpoint inhibitor therapies. Tebentafusp is a first-in-class bispecific fusion protein (ImmTAC) composed of an affinity-enhanced T cell receptor targeting the gp100 peptide presented by HLA-A*02:01 molecules on melanoma cells, fused to an anti-CD3 single-chain variable fragment that redirects and activates CD3+ T cells to kill tumor cells. Durvalumab is a monoclonal antibody immune checkpoint inhibitor targeting programmed death ligand 1 (PD-L1), blocking its interaction with PD-1 receptors, thereby promoting T cell-mediated anti-tumor immunity. Together, this combination aims to simultaneously facilitate T cell recruitment and activation at the tumor microenvironment (via tebentafusp) and release immune inhibition (via durvalumab). Clinical studies have shown promising efficacy and safety in heavily pretreated patients, with response rates and overall survival rates encouraging in this patient population[1][3][4].
