tebipenem

Tebipenem is an oral carbapenem antibiotic that represents a significant advancement in antimicrobial therapy as the first orally available carbapenem. It is primarily being developed for complicated urinary tract infections (cUTI) and acute pyelonephritis (AP) in adults. Tebipenem is formulated as tebipenem pivoxil hydrobromide (TBP-PI-HBr), a prodrug that improves oral bioavailability. The active moiety, tebipenem, belongs to the carbapenem subclass of β-lactam antibiotics, valued for their broad spectrum of activity, including against drug-resistant pathogens. Tebipenem works by binding to penicillin-binding proteins, disrupting bacterial cell wall synthesis, making it effective against extended-spectrum β-lactamase (ESBL) producing Enterobacterales. It was first approved in Japan in 2009 under the brand name Orapenem for pediatric use in ear, nose, throat, and respiratory infections.
