Technetium (99mTc) tetrofosmin is an approved diagnostic radiopharmaceutical used as a SPECT imaging agent for myocardial perfusion imaging (MPI). It consists of the radioisotope technetium-99m chelated by two tetrofosmin ligands (1,2-bis[di-(2-ethoxyethyl)phosphino]ethane). Following intravenous administration, the lipophilic cationic complex is rapidly taken up by myocardial tissue in proportion to regional blood flow and sequestered within mitochondria. It is primarily indicated for the delineation of reversible myocardial ischemia and infarcted myocardium in patients with known or suspected coronary artery disease, as well as for the assessment of left ventricular function.
