Technetium-99 ethane-1-hydroxy-1,1-diphosphonate (99Tc-EHDP) is a therapeutic radiopharmaceutical complex consisting of the radionuclide technetium-99 and the bisphosphonate carrier etidronate (EHDP). Unlike the metastable technetium-99m used in diagnostic bone scintigraphy, this compound utilizes the long-lived technetium-99 isotope for therapeutic applications in bone and joint disorders. The drug exhibits a high affinity for hydroxyapatite crystals in the bone matrix, concentrating in areas of active bone remodeling, high metabolic turnover, or impaired vascularity. It is currently being investigated by Shanghai Tenth People's Hospital in China for the treatment of ischemic necrosis of the lunate bone (Kienböck's disease), where it is intended to facilitate bone repair and revascularization. The therapeutic effect is attributed to the combination of the bisphosphonate's regulatory action on bone metabolism and the potential anti-inflammatory properties of the technetium complex.
