Technetium-99m certolizumab pegol is an investigational radiopharmaceutical developed by researchers at Ghent University Hospital (Universitair Ziekenhuis Gent) for the molecular imaging of inflammation. It consists of the TNF-alpha inhibitor certolizumab pegol, a PEGylated humanized Fab' fragment, labeled with the gamma-emitting radioisotope technetium-99m (99mTc). This agent is primarily used in scintigraphy (SPECT imaging) to visualize the distribution and expression of Tumor necrosis factor (TNF) in vivo. Its clinical application is focused on patients with axial spondyloarthritis (axSpA) and rheumatoid arthritis to identify active inflammatory lesions and potentially predict clinical response to TNF-inhibitor therapy. By binding specifically to TNF, the tracer allows for the non-invasive assessment of local cytokine levels in joints and the spine.
