Technetium-99m glucoheptonate (99mTc-GH) is a small molecule radiopharmaceutical diagnostic agent used in nuclear medicine for imaging the kidneys, brain, and cardiac blood pool. It consists of the metastable radioisotope Technetium-99m chelated by glucoheptonate ligands. In the Philippines, the Philippine Nuclear Research Institute (PNRI) produces this agent through its Tc-99m Generator Facility for domestic clinical use. The drug is primarily employed to evaluate renal perfusion and function, diagnose acute pyelonephritis, and quantify split renal function. In neurology, it is used to image intracranial lesions where the blood-brain barrier is disrupted, such as brain tumors. Its mechanism of action involves active concentration in renal proximal tubules via organic anion transporters (OATs) and uptake in tumors or inflamed tissues, which is believed to be mediated by glucose transporters like GLUT-1 and GLUT-4.
