Overview

Technetium-99m hexamethylpropylene amine oxime (HMPAO), also known as technetium Tc‑99m exametazime, is a radiopharmaceutical diagnostic agent. It consists of the chelating agent exametazime (HMPAO) complexed with the radioisotope technetium‑99m. The compound is used primarily in nuclear medicine for scintigraphic imaging to detect altered regional cerebral perfusion, particularly in stroke and other cerebrovascular diseases. Additionally, it is used for labeling leukocytes to localize intra-abdominal infections and inflammatory bowel disease. The mechanism of action involves the lipophilic nature of HMPAO, which allows it to cross cell membranes and the blood-brain barrier; once inside cells, it binds to mitochondria and nuclei, enabling imaging of tissue perfusion or infection sites[2][4][6].
