Technetium-99m sestamibi is a radiopharmaceutical diagnostic agent used primarily in nuclear medicine for myocardial perfusion imaging to detect coronary artery disease and evaluate myocardial function. It consists of the radioisotope technetium‑99m complexed with six methoxyisobutylisonitrile (MIBI) ligands. After intravenous injection, it is taken up by tissues with high mitochondrial content and negative plasma membrane potentials, such as myocardium, parathyroid tissue, and certain breast tissues. Its uptake in myocardial cells reflects blood flow and mitochondrial activity; areas of reduced uptake indicate ischemia or infarction. The compound is also used for preoperative localization of parathyroid adenomas in hyperparathyroidism and as an adjunct in breast cancer diagnostics through scintimammography or breast-specific gamma imaging[1][2][5][6][8].
