Technetium 99Tc methylene diphosphonate is a radiopharmaceutical developed by Chengdu Yunke Pharmaceutical Co., Ltd. It functions as an anti-inflammatory and anti-bone destruction agent. Its mechanism of action involves inhibiting macrophage infiltration and downregulating proinflammatory cytokines such as tumor necrosis factor-alpha (TNF-α) and interleukin-1 beta (IL-1β). Additionally, it exhibits anti-osteoclastogenic activity, promotes osteoblast proliferation, and influences the OPG/RANKL/RANK system by increasing osteoprotegerin (OPG) expression and decreasing receptor activator of nuclear factor kappa-B ligand (RANKL) expression. It also binds to hydroxyapatite crystals in bone. Primarily used in China, it has been marketed for the treatment of rheumatoid arthritis and osteoporosis.
