Technetium Tc 94m sestamibi is a radioconjugate consisting of the synthetic molecule sestamibi—an isonitrile compound—labeled with the positron-emitting isotope technetium-94m. Sestamibi itself is a lipophilic cationic agent that accumulates in tissues with high mitochondrial content and negative membrane potentials. When labeled with technetium isotopes, it serves as a radiotracer for imaging applications. The mechanism of action involves passive diffusion into cells and electrostatic retention within mitochondria; however, the precise mechanism for tissue localization can vary by tissue type and has not been fully established. The primary use of technetium-labeled sestamibi compounds (such as those using other isotopes) includes myocardial perfusion imaging to detect coronary artery disease, parathyroid adenoma localization, and breast cancer work-up[7]. With the positron-emitting isotope Tc-94m specifically, it would be used in PET imaging contexts.
