Technetium Tc-99m dextran is a diagnostic radiopharmaceutical agent used in nuclear medicine for various imaging applications, including lymphoscintigraphy, detection of protein-losing enteropathy (PLE), and assessment of vascular permeability. It consists of the metastable, gamma-emitting radionuclide technetium-99m (99mTc) complexed with dextran, a branched polysaccharide. The agent's biodistribution and retention within specific fluid compartments are primarily determined by the molecular weight of the dextran carrier; high-molecular-weight formulations (e.g., ~81,000 Da) tend to remain within the intravascular space, making them suitable for blood-pool imaging, while other formulations are evaluated for visualizing lymphatic flow and identifying sentinel lymph nodes in breast cancer patients. Its mechanism of action relies on passive distribution within fluid compartments rather than specific receptor binding, allowing for detection by gamma cameras while minimizing radiation exposure due to the short half-life of 99mTc.
