99mTc-glucarate is a technetium-99m labeled radiopharmaceutical developed by Molecular Targeting Technologies, Inc. (MTTI) as a SPECT imaging agent for the detection of acute myocardial injury and ischemia. The tracer consists of the radioisotope technetium-99m chelated with glucarate, a six-carbon dicarboxylic acid. Glucarate exhibits a high affinity for histones and other positively charged intracellular proteins that become accessible following the loss of cell membrane integrity in necrotic or severely ischemic tissues. Unlike traditional "cold spot" imaging agents like thallium-201 or Tc-99m sestamibi, 99mTc-glucarate acts as a "hot spot" imaging agent, accumulating specifically in damaged myocardium within a narrow window (typically 1-9 hours) after the onset of symptoms. It is primarily investigated for use in the emergency department to differentiate acute coronary syndrome (ACS) from non-cardiac chest pain, particularly in patients with non-diagnostic EKGs.
