Technetium Tc-99m neomannosyl human serum albumin (99mTc-MSA) is a radiopharmaceutical agent specifically designed for sentinel lymph node (SLN) imaging and mapping. It is composed of human serum albumin (HSA) conjugated with multiple mannose molecules (typically around 15-16 per albumin molecule) and labeled with the radioisotope technetium-99m. The drug's mechanism of action relies on its high affinity for mannose receptors (CD206), which are abundantly expressed on the surface of macrophages and dendritic cells residing within lymph nodes. Following intratumoral or submucosal injection near a primary tumor, 99mTc-MSA is transported via the lymphatic system to the sentinel lymph nodes, where it is internalized by receptor-mediated endocytosis. The emitted gamma radiation from the technetium-99m label allows for the intraoperative detection and localization of these nodes using a gamma probe, facilitating accurate nodal staging in cancers such as non-small cell lung cancer and esophageal cancer.
